Aftercare and the FOY Alumni Community

Support That Continues After PHP and IOP in Agoura Hills, California

Finishing treatment is a milestone, and it is also the start of a new chapter. Recovery does not end when a program does, and the weeks and months after discharge are when having support matters most. At FOY Wellness and Recovery, you do not leave the community when you leave the program.

Once a part of FOY, you are always a part of FOY.

Why Aftercare Matters

Treatment gives you tools. Aftercare helps you keep using them in real life. After PHP or IOP, you return to a full routine of work, family, and everyday stress, often without the daily structure and support you had in the program. Ongoing connection can help you stay accountable, notice warning signs early, and remember you are not doing this alone.

Recovery from mental health conditions and substance use is a long-term process for many people. A strong aftercare plan looks different for everyone, and it usually includes several kinds of support working together.

The FOY Alumni Monthly Community Meeting

Our alumni community meets once a month. It is a welcoming space to reconnect, hear from peers, share what is working, and talk openly about what is hard.

What you can expect:

  • A supportive, nonjudgmental group of people who understand recovery
  • A chance to celebrate milestones and encourage one another
  • Connection with FOY staff and fellow alumni

This meeting is a community gathering. It is not a substitute for therapy, medical care, or a recovery program such as 12-step or other mutual-support groups, and it is not an emergency service.

Your Aftercare Plan

Aftercare planning begins before you finish treatment. As you step down from PHP to IOP and prepare for discharge, your clinical team works with you on a plan that fits your life. Depending on your needs, a plan may include:

  • Continued therapy. Weekly individual therapy with a provider near your home, or continued sessions with a trusted clinician.
  • Mutual-support groups. Peer groups such as 12-step meetings, SMART Recovery, or other community supports that match your values.
  • Healthy routines. Sleep, movement, nutrition, and mindfulness practices you began in treatment, such as yoga, breathwork, or meditation.
  • Family and relationship support. Involving loved ones where appropriate, with your consent.
  • The FOY Alummi community. Monthly connection with people who have walked a similar path.

Staying Connected Between Meetings

Recovery has ups and downs, and you do not need to wait for a meeting if you need help.

If You Are Struggling After Treatment

Setbacks can happen, and they do not erase your progress. If you notice old patterns returning, you can reach out for support at any time. If you need a higher level of care again, our team can help you evaluate your options. Returning to treatment is a sign of commitment to your recovery, not failure.

Insurance and Returning to Treatment

If you or someone you love needs PHP or IOP, FOY is in-network with Blue Shield, Cigna, and TRICARE through TriWest, and we work with out-of-network PPO plans including Anthem, Aetna, Multiplan, HMC HealthWorks, and Blue Cross Blue Shield. We verify your benefits at no cost.
